Family Connects NJ is changing the way postpartum care is delivered to families across the state. Every family caring for a newborn, or recovering from a birth can benefit from a nurse visit at home.
Family Connects NJ is a new program that connects parents with a specially trained nurse for a personalized follow-up visit at home within the first two weeks after their child’s birth. Visits are offered to all families at no cost, regardless of income, insurance, or immigration status.
All New Jersey families who have welcomed a newborn into their lives, whether through birth, adoption, or as a resource parent or kinship caregiver, can receive the support they may need as they navigate the challenges, joys, and adjustments of parenthood. Whether this is your first child or one of many, every parent can use a little extra support – and Family Connects NJ is ready and willing to serve you.
Our mission is to support the well-being of all families by delivering care from specially trained, registered nurses in the comfort of the family home. During the Family Connects NJ visit, our nurses assess the health of both the parents and the newborn and offer valuable information during this important transition. Covered areas can include feeding and sleeping tips, connecting parents to helpful resources in their community, or assisting families recovering from the tragedy of stillbirth.
The Benefits of Family Connects NJ
- Care is provided at no cost to you in the comfort of the family home and is completely voluntary.
- Services are available to birth, adoptive, and resource families with a newborn as well as parents experiencing a stillbirth or loss of their newborn.
- Specially trained registered nurses evaluate health, screen for potential complications, answer questions about welcoming a newborn, and address any unexpected changes families are experiencing.
- Nurses will connect families to community resources based on their specific circumstances.
